Waiver
I understand that volunteering may require physical exertion. I acknowledge that injury may result during instruction, including but not limited to as a result of physical contact with other students, instructors, or fixed or movable objects.
I agree to assume all risks associated with my child participating in volunteering. I represent that my child is medically and physically able to participate in an acting and movement class. In view of the foregoing, and as a term and condition of receiving direction from The Service Collective and its representatives. I, my spouse, and legal representatives expressly release Amanda Jones, The Service Collective,, and the physical facility from all liability.
Behavior
By registering for The Service Collective programming, I acknowledge that The Service Collective has the right to request a caregiver/designated adult accompany my minor child for the full duration of an event if they are displaying dangerous behavior to themselves, our teachers, or other children. This includes, but is not limited to, persistently leaving the activity area or language not suitable for all audiences.
In the event that I cannot provide a designated adult to accompany my child for their volunteer event to provide a safe environment for the group as a whole, I understand that my child will not be permitted to attend the activity.
Media Release
I consent to the publication and use of my name and/or likeness for the purpose of promotion, publicity, advertising or trade by the The Service Collective (TSC) or anyone authorized by the TSC to act on its behalf. “My likeness” includes photographs, sound and/or video recordings, films, broadcasts, brochures, publications, reports, web pages, promotional materials or any other audio-visual, electronic, printed, tangible work in any media or format, now known or hereafter to become known, and/or reproductions of any of these. I agree that the actual material involved is and shall continue to be the property of TSC and that I shall have no right of review or approval regarding the use of my name and/or likeness in such material.
I understand and agree that I will not be compensated in any way for the use of my name and/or likeness by TSC.
I represent that I am at least 18 years of age or I represent and warrant that I am the parent or legal guardian of the above-signed minor (“Minor”). I have read the foregoing Consent and am familiar with all of the terms and conditions thereof and I consent to its execution by the Minor. I agree that neither the Minor nor I will revoke or disaffirm the foregoing agreement at any time. I agree to indemnify and hold TSC and its agents, directors, officers, consultants, employees and other volunteers and trainees harmless from and against any and all claims, liabilities, costs or expenses, including reasonable attorneys’ fees, that may arise from the breach or alleged breach by the Minor or me of the foregoing Consent or this Agreement.
